Formula:

S + to be (was, were) + being + V3 + by + O

Example:

> active voice = Mr. Smith was cutting the paper.

                          (tuan smith sedang memotong kertas)

> passive voice = the paper was being cut by Mr. smith.

                          (kertas itu sedang dipotong oleh tuan smith)
